SUMMER ENRICHMENT COURSE REGISTRATION FORM

Your child MUST be pre-registered for all courses!  We “sold out” last year with waiting lists, so please register early! We have a maximum capacity of 25 students per course.  Once we are at capacity for a course, we will start a waiting list.  A $45 non-refundable deposit per course is required upon registration and the balance due one week prior to the start of the course.  Please mail this form (may be copied) with your check to NC, ATTENTION:  Deb Matson.  Typically, it is the Study Skills/Junior High Success courses that have a waiting list.  If the Day School course(s) do not have sufficient enrollment, then there may be an option of having the Study Skills/Junior High Success course during those weeks instead for students on the wait list!  We will advise everyone on the wait list as to the status of the program.  More information will be sent to you in the upcoming weeks.
